Provider Notes:	I have worked for the State of Oregon for 5 years as a Social Service Asst. I conducted visitations with transport of children then documented the visitations. I am a 16-year veteran foster care provider, I have 4 adopted daughters, and have worked in Washington doing supervised visitations for 6 years, working for Catholic community services for 3 of those years also conducting visitations. I am currently doing supervised visitations for Kid Stuff Family Monitoring and have been a subcontractor with this company since January 2021.
